This form is for requesting a DVD or CD copy of Chapel or other NWC event. Only certain events are available, including limitations on content protected by copyright. Requests are normally completed within 72 hours. Payment will be received at the Learning Resource Center Desk when picking up item(s). Other items may be brought to the LRC Desk for copying (VHS, MiniDV, CD, DVD, Cassette). Material protected by Copyright may not be copied without written consent of the Copyright owner.
